Hospice Community Liaison

: Blize Healthcare is looking for a Hospice Community Liaison to join our team. You will develop and maintain relationships with referral sources, primarily healthcare providers and community organizations, to promote hospice services and increase patient referrals. You will focus on building rapport, educating about hospice care, and facilitating the referral process. Responsibilities:

• Establishing strong relationships with physicians, hospitals, nursing facilities, and other referral sources.
• Seeking and securing new referrals by educating potential referral sources about hospice services and benefits.
• Implementing marketing strategies, attending trade shows, and participating in community events to raise awareness of hospice services.
• Informing the community about hospice/home health care, services, and benefits.
• Tracking and reporting referral and admission data to identify trends and growth opportunities.
• Working with the clinical team to ensure a smooth transition to hospice/home health care.

Required Skills and Qualifications:

• Strong communication and interpersonal skills.
• Excellent presentation and sales skills.
• Ability to build and maintain relationships.
• Understanding of the hospice care continuum and related regulations.
• Experience in healthcare marketing or sales is beneficial.
• Associate's degree or equivalent experience.
