We are seeking an experienced and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our primary care clinic focused on value-based, relationship-driven care. This role is ideal for an APP who believes great primary care is built on time, trust, prevention, and long‑term outcomes—not rushed visits and volume-driven metrics. In our clinic, APPs are trusted primary care providers with their own patient panels, supported by interdisciplinary teams and a care model designed to keep patients healthy, engaged, and out of the hospital.
