Accompany each resident on all medical visits and stay until the visit is complete. Sign in resident upon entrance and present resident’s documents to office personnel. Obtain resident’s documents, new prescriptions, and doctor’s order form; collect appointment cards if a follow‑up is necessary. After returning to the facility, report to RCM and be prepared to answer questions concerning the visit. Perform other duties as required and assigned. Qualifications Physically able to help ambulatory and non‑ambulatory residents transfer to and from wheelchair and vehicle. Possess a valid state driver’s license. Qualified as a Certified Nurse Assistant (C.N.A.). Maintain a clean driving record. Maintain a clean criminal record. Hold any other state‑required certifications.
