Shift: 5x8 evenings (8-hour shifts), 15:00–23:30

Responsibilities
- Administer prescribed medications and treatments in accordance with physician orders and facility policies
- Monitor residents’ health status, document assessments, and promptly communicate changes to the care team
- Provide support with activities of daily living and assist with mobility, hygiene, and comfort measures
- Collaborate with physicians, RNs, and other team members to implement and update individualized care plans
- Maintain accurate nursing documentation, MARs, incident reports, and shift handoffs
- Ensure a safe, clean, and respectful environment; adhere to infection control and safety protocols

Requirements
- Active Licensed Practical Nurse license in Pennsylvania
- Graduation from an accredited LPN program
- Prior experience in a long-term care or skilled nursing setting preferred
- Ability to work evenings and every other weekend as required
- Proficiency with basic charting and electronic health records; reliable and detail-oriented
- Capacity to work with an aging population and manage multiple residents with varying needs
