Patient Care Manager, Ambulatory Nursing

Responsibilities Patient Care Manager provides onsite leadership and operational support to frontline nursing teams across multiple clinic locations, partnering closely with providers, ambulatory leadership, and interdisciplinary teams to ensure safe, efficient, and high-quality care delivery. Coordinated patient care, supervision of a professional nursing team, administrative and budget management, troubleshooting of problem areas, organizational change management, ensuring compliance with accreditation and regulatory standards, and quality assurance/quality improvement. Ensures the effective delivery of competent, compassionate, and efficient patient care by planning, organizing, and directing nursing services, implementing physicians' orders, and monitoring patient care processes and outcomes. Demonstrates clinical competency in assigned specialty area, role modeling nursing practice consistently with the organizational and department mission, vision and values. Communicates in a clear and effective manner to enhance team effectiveness, manage conflicts, engage employees, and interdisciplinary colleagues in unit activities and performance changes. Plans and forecasts the department budget, interprets financial performance, identifies key drivers, and collaborates with physician and nursing colleagues on unit and capital budget needs. Directs personnel management of employees, ensuring new staff is hired, trained, oriented, precepted and evaluated, and participates in employee performance appraisals and disciplinary actions. Assumes 24‑hour responsibility and accountability for the operation of the assigned patient care unit(s), plans and directs short and long term departmental activities in accordance with hospital goals and accrediting organization standards. Develops, implements, and evaluates process improvement projects/process to continually monitor and improve patient care, and achieves patient and staff satisfaction targets for the organization and unit of responsibility. Facilitates professional development and clinical growth of nursing staff through implementation and maintenance of unit specific competencies, and promotes educational opportunities based on assessment of staff needs. Supports the quality framework, creates an environment of patient care that is safe, effective, patient centered, timely, efficient and equitable. Qualifications Education: Bachelor of Science in Nursing (BSN) from an accredited college or university. Experience: Three (3) years of progressively responsible and directly related work experience. License/Certification: Registered Nurse by California Board of Registered Nursing. Knowledge: Ability to develop programs and lead process improvement projects, supervise, coach, mentor, train, and evaluate work results, and apply leadership to meet patient needs and achieve shared goals.
