Physician Leader Primary Care

HarmonyCares seeks a Physician Leader Primary Care to deliver and oversee home-based primary care for medically complex, home-limited adults. You will provide direct clinical care, lead an interdisciplinary team, and ensure evidence-based, patient-centered practice. Responsibilities include supervising clinicians, coordinating in-home and telehealth visits, optimizing chronic disease management, and reducing avoidable hospitalizations. You will champion quality, safety, and regulatory compliance while mentoring providers, supporting palliative care integration, and partnering with families to align care with patient goals in a mission-driven environment.
