Job Details

• Weekly Estimated Pay: $2,030 - $2,190 (competitive weekly pay for travel PT jobs)
• Location: Fort Morgan, CO (Home Health & Hospice) - Fort Morgan, Colorado 80701
• Contract Duration: 9 weeks (March 30, 2026 - May 30, 2026)
• Shift: Monday-Friday, 8:00 AM - 4:30 PM
• Hours per Week: 40 (Guaranteed)
• Coverage Area: Up to 50-mile radius from Fort Morgan
• On Call/Weekend Requirement: None
• Travel: Mileage reimbursed between patient visits

Qualifications / Job Requirements

• Master's or Doctorate in Physical Therapy (MPT or DPT required)
• Valid Physical Therapist license for Colorado (CO PT license)
• Current BLS certification (AHA preferred)
• Minimum 1 year of experience as a Home Health OASIS clinician or equivalent home health PT experience
• Strong documentation skills (OASIS, visit notes, reassessments, discharge summaries)
• Experience with HCHB (Homecare Homebase) is a plus
• Reliable vehicle and valid driver's license for travel between patient homes
• Up-to-date vaccinations, including TB testing
• Blue/black scrubs required

Primary Responsibilities

• Provide skilled physical therapy services in the home setting focusing on rehab, functional mobility, and safety
• Conduct comprehensive patient evaluations to assess mobility, strength, balance, pain, and fall risk
• Develop and implement physician-ordered Plan of Care with measurable goals and interventions
• Deliver therapeutic exercise, gait and balance training, transfer training, pain management, and fall prevention strategies
• Complete accurate, timely OASIS documentation and maintain visit notes, reassessments, and discharge summaries for regulatory compliance
• Communicate patient status and progress with physicians and interdisciplinary care team members to support coordinated home health care
