Responsibilities
- Perform initial and ongoing PT evaluations for residents in the long-term care setting.
- Develop and implement individualized treatment plans with measurable goals.
- Provide evidence-based interventions (therapeutic exercises, gait and balance training, functional mobility, pain management) to improve or maintain function.
- Document progress, update care plans, and communicate with the interdisciplinary team.
- Reassess progress regularly and adjust plans of care to meet resident goals and regulatory requirements.
- Ensure safety, quality of care, and compliance with policies; participate in discharge planning and teamwork with nursing and therapy colleagues.

Requirements
- Active Physical Therapist license in New Mexico (or eligible per state requirements).
- Doctor of Physical Therapy (DPT) degree from an accredited program.
- CPR/BLS certification.
- Experience in geriatric rehabilitation or skilled nursing care preferred.
- Strong communication and collaboration skills; ability to work with a multidisciplinary team.
- Ability to meet guaranteed 38 hours per week and adapt to a travel assignment schedule.
