Licensed Practical Nurse

We have multiple full-time and part-time 8 hour shifts available.We are currently hiring for the following locations:Essex County Correctional Facility in Newark, NJHudson County Correctional Facility in Kearny, NJ.Job Summary:In a team relationship with the Registered Nurse, the Licensed Practical Nurse contributes to the nursing process through on-going systematic data collection, assisting with the health assessment, recording observations, goal identification and patient health education.Duties / Responsibility:Patient CareCollaboration with Healthcare teamHealth and Safety complianceClinical tasksSkills / Abilities:Critical thinking and problem solvingCommunicationAttention to detailTime management and organizationEmpathy and compassionPhysical stamina and strengthAdaptability and flexibilityCultural competencyEthics and professionalismPerformance Expectations:In a team relationship with the RN supervisor, contributes to nursing assessment, care and planningInitiates the admission process and assists with the identification of recurrent health problems, symptoms and behavioral changes of the patientCollects pertinent data through observation, interview, other members of the health team and communicates changes in the patient condition to other patient care providersRecords all data obtained through assessment of the patient’s health recordAssists in the formation of a written planAssists in identifying both short and long term timed, measurable goals with anticipated behavioral changesAssists in setting priorities when planning care to meet patient needsAssists with patient health care and participates in multi-disciplinary care conferencesCounts CDS Drugs and contraband, if requiredAssists physician in minor medical/surgical proceduresTranscribes and posts all therapeutic orders generated by the physicianInitiates and documents prescribed treatments for nursing interventions in compliance with policy and procedureAdministers routine and/or PRN medications as orderedPractices universal precautions when providing nursing proceduresAssists in maintaining a neat, clean and safe environment for patients and co-workersKeeps patient information confidential and respects patients’ right to privacyKeeps health team informed of observations and changing conditions of patientsMust maintain and show bi-annual proof of active state nursing licensure with a copy to be placed in the personnel file maintained by the RN SupervisorWill be neat in appearance and adhere to nursing code policy; shall wear a white lab coatArrives promptly for scheduled tour of duty for verbal report from off-going shiftIf requested by the RN Supervisor or Administrator, will present a doctor’s excuse if absent the day before or the day after a previously scheduled two days offWill notify the RN Supervisor if becoming ill on duty to notify him/her of the dispositionSubmits request for vacation or special time-off in advance of the posting of scheduled timeArrange workday exchange in the same pay period with a comparable member of the nursing staff for the desired day off if the schedule has already been postedAfter the agreement is reached, the Charge Nurse and/or the RN Supervisor are to be notified in writing by both parties of the desired schedule change agreementAll approved schedule modifications will be at the discretion of the Charge Nurse and/or the RN SupervisorIs cognizant and adheres to all security regulations of the institutionAdheres to assigned break and lunch periodsRemains at assigned post until properly relievedAttends scheduled in-service programs and staff meetingsMaintains an active Hands-On CPR certificationAll other duties as assignedRequired QualificationsGraduate of an accredited Practical Nursing ProgramCurrent, active Licensed Practical Nurse (LPN) license in the state of practiceCurrent CPR/BLS certificationKnowledge of nursing principles, practices, and proceduresAbility to follow clinical protocols, physician orders, and regulatory standardsStrong communication, interpersonal, and teamwork skillsPreferred QualificationsPrevious experience in acute care, long-term care, behavioral health, or correctional healthcareProficiency with electronic medical record systems (EMR/EHR)IV certification (if applicable and permitted by state regulations)Organizational ExpectationsProvides a positive and professional representation of CFG Health Systems, LLC.Promotes a culture of safety for patients and employees through proper identification, reporting, documentation, and preventionMaintains competency and knowledge of current standards of practice, trends, and developments in related scope of job role or practiceAdheres to infection control policies and protocols, medication administration and storage procedures, and controlled substance regulationsParticipate in ongoing quality improvement activitiesCompletes required orientation as directed by the facilityFollows facility and OSHA safety rules and procedures while on assignmentFollows facility and CFG Health Systems Occurrence ProtocolUpholds HIPAA and PREA regulationsPunctual and dependable for assigned/confirmed shiftsMaintains security clearance for the assigned facilityPhysical DemandsThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
