PT - Physical Therapist

Outpatient Physical Therapist

This is a 12-week contract position, starting August 1st.

Full-time, Monday through Friday, 40 hours per week.

The Outpatient Physical Therapist will evaluate all assigned patients and develop, implement, and modify treatment plans in accordance with patients' needs, goals, and physician direction. This role is responsible for the delegation and direction of all discipline-specific therapy services for assigned patients. The therapist will ensure the design of functional treatment programs allowing for effective return to community activities and serve as an educator for patients, families, students, coworkers, and self.

Responsibilities:

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop treatment plans and goals focused on progressing patients towards discharge within an established length of stay.
• Document medical records in a clear, concise, accurate, and timely manner.
• Ensure care is provided under signed, valid physician orders and within insurance approval.
• Participate in the student program, providing a positive learning environment.
• Actively pursue professional development through continuing education and involvement in professional activities.
• Adhere to discipline-specific State Practice Act and maintain professional licensure.
• Demonstrate understanding of performance improvement initiatives to improve service quality.

Required Experience / Certifications / Licensure:

• Bachelor's or higher degree in Physical Therapy from an accredited school.
• Minimum of 1 year of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state license to practice Physical Therapy.
