Job Summary:

Seeking an experienced RN in Case Management to provide comprehensive patient coordination and care planning. This travel role offers an opportunity to work within a fully accredited healthcare facility dedicated to high-quality, cost-effective patient outcomes.

Responsibilities:

  Assess patient needs and develop individualized care plans aligned with clinical goals and regulatory standards.

  Collaborate with multidisciplinary teams to coordinate services, ensuring seamless patient transitions across care settings.

  Maintain accurate documentation of patient progress, services provided, and discharge planning activities.

  Monitor and evaluate patient outcomes, adjusting care plans as needed to promote recovery and optimal health.

  Ensure compliance with hospital policies, state regulations, and accreditation standards related to case management.
  
  

Qualifications:

• 
  Valid RN license with at least 2 years of recent hospital case management experience.

  Proven knowledge of HCA protocols and familiarity with the MIDAS facility overview.
  
  
• 
  Strong clinical assessment skills and ability to develop effective care strategies.
  
  
• 
  Excellent communication skills, with the capability to collaborate effectively with healthcare teams and patients.

  Demonstrated ability to manage complex cases and prioritize care activities efficiently.
