Required Qualifications

Occupational Therapist, Pedi OR Occupational Therapist, Outpatient Licenses: OT-IL Certifications: BLS(Copy Needed) SSN Required DOB Required 1 reference from the past 12 months. No local candidates, must be 50 miles away from Facility experience working independently graduate of an accredited OT program
