Hospice RN Case Manager — Home-Based Care

Hospice Plus seeks a compassionate Hospice RN / RN Case Manager to deliver and document skilled nursing care in patients' homes or wherever they reside. You will perform initial and ongoing assessments, collaborate with an interdisciplinary team, and provide education and bereavement support to families. Requirements include Texas RN license and at least 1 year of RN experience; hospice or palliative care preferred. This is a full-time position with comprehensive benefits. #J-18808-Ljbffr Gentiva
