Orientation $250-262.50 - half the hourly/callback rate, kicks in after 8 hours. "Per hour of orientation or training beyond 8 hours"

Hourly $500-$525 - "Per hour, not OT"

Holiday $750-787.50 - Must be 1.5x the hourly rate. "Per holiday hour, no OT"

Nightly Call $1300-2000 - "Per night call to hold the pager, to include 2.5 hours gratis"

Holiday Nightly Call - $1950-3000 - "Per night call to hold the pager, to include 2.5 hours gratis"

On Call (Daily) $4200-4500 - "Per 24 hours to include 4 hours gratis"

24 Hour Holiday Call Rate - "Per 24 hours to include 4 hours gratis"

Call Back $500-$525 "Per hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms."

Holiday Call Back $750-787.50 "Per holiday hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms."

Schedule/Availability Requirements:

Availability: ASAP, July and ongoing Schedule - needed weekly:

• Friday: 5p - 7a (site willing to use provider for day shift on Friday, if provider is available to start earlier)
• Saturday: 7a - 7a
• Sunday: 7a - 7a

Facility Location: Mercy Council Bluffs Setting: Hospital setting performing 3,600 IP/OP surgeries per year. Duties: Independent Cases FTE: 1.0 Required Procedures: Ortho, Labor & Delivery, Spine, GI, Gynecology, Urology, Cath Lab, Interventional Radiology, bariatric bypass, minimal vascular cases EMR: EPIC Support Staff: Anesthesia providers, CRNA, Anesthesia Tech Reason for Coverage: Staffing Shortage Deal breakers: N/A
