Pediatric Clinical Pharmacist - Neurology

Job Summary Provides leadership in a specialized healthcare team to assure optimal cost‑effective drug therapy. Leads efforts in prevention and reporting of medication errors and adverse drug reactions, continuously improving processes and practice to enhance medication safety. Position Highlights The Pediatric Neurology Clinical Pharmacist works directly with Neurology providers and patients, providing comprehensive pharmacy services in the outpatient setting. Responsibilities include medication reconciliation, pharmacotherapy optimization, patient counseling, prior authorization review and approval, retail prescription assistance, and outpatient pharmacy navigation. Collaboration and integration with pharmacy, nursing and medical staff, along with excellent communication skills, are essential. Completion of an ASHP accredited PGY‑1 residency program in a pediatric hospital setting or two years of pediatric hospital experience is preferred. Key Responsibilities Review drug orders and prescriptions to ensure safe, appropriate, and cost‑effective medication use. Prepare and dispense medications, provide drug information, and manage medication therapy per federal, state, local regulations and hospital policy. Optimize medication preparation and resolve medication‑related problems. Maintain quality, safety, and consistency of clinical pharmacy services in assigned areas. Supervise support personnel, verifying accuracy and appropriateness of all medications before dispensing to patients or providers. Technical Capabilities Mentoring & Coaching (Intermediate): Demonstrates good training and mentoring practices, encourages two‑way dialogue, and explores alternative solutions. Medication Management (Advanced): Assesses clinical appropriateness, dose, indication, contraindications, potential adverse effects, and interactions; monitors lab results and provides drug information. Pharmaceutical Drug Preparation & Dispensing (Novice): Proficient in reviewing, preparing, and dispensing prescribed medications and in aseptic technique for sterile preparations where applicable. Pharmaceutical Quality Assurance & Compliance (Expert): Leads quality improvement, ensures compliance with regulations, and may represent the organization in regulatory discussions. Pharmaceutical Computer Literacy (Novice): Demonstrates proficient use of pharmaceutical computer applications for moderate‑difficulty patient care issues. Position Qualifications Licensing: Pharmacist license in Tennessee. Experience: 1 year of relevant pharmacy experience. Education: Bachelor’s degree (required).
