Requirements:

Must be Board certified in Orthopedic surgery or a Truly board eligible - [REQUIRED]
Have an active license in both Iowa and Illinois - [REQUIRED]
Strong clinical and surgical expertise with a commitment to comprehensive hand and upper extremity care - [REQUIRED]
Open to graduating residents/fellows and experienced surgeons
Does the provider require visa support – H-1B cap exempt (Note in Additional Submission Details) – [REQUIRED]
Submit Self-Query NPDB pulled within 30 days from submission at time of presentation- [REQUIRED]
Active Iowa medical license
Active Illinois medical license
Board Certified
Board Eligible
