Minimum Requirements
Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
Active Illinois Physical Therapist license required to start the assignment.
BLS certification from AHA or ARC may be required.
Experience working in skilled nursing facility settings preferred.
Experience managing caseloads in post-acute or long-term care environments preferred.
Willingness to split assignment and travel between multiple local facilities (Tuscola, Arcola, Bement) as required by the client.
Ability to work a full-time schedule and meet facility documentation and productivity expectations.

Assignment Details
Facility Type: Skilled Nursing Facility.
Shift Name: Days.
Shift Details: Days 08:30-16:30, 5x8 schedule.
Assignment Structure: Split assignment between multiple local facilities within the Tuscola area; travel between sites required.
Contract Weeks: 13-week assignment.
