Physical Therapist

Minimum Requirements

• Bachelor‘s, Master‘s or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA. Minimum education requirement: Master‘s preferred by client.
• Active West Virginia Physical Therapist license required to start the assignment.
• BLS Certification may be required from AHA or ARC.
• Previous home health experience preferred. Client will consider candidates with experience in home health settings or equivalent outpatient/skilled nursing exposure.
• Ability to cover Fayette County and Nicholas County geographic areas. Reliable transportation required for travel between patient visits.
• Willingness to participate in on-call rotation as call responsibilities decrease with additional PT staff.
• Professional certification required by client. Maintain required professional credentials throughout assignment.
• Dress code: Navy blue scrubs.

• Facility Type: Home Health.
• Shift Name: Days.
• Shift Details: Days.
• Weekly Hours: 32 hours per week guarantee.
• Coverage Areas: Fayette County and Nicholas County service areas.
• Productivity expectations: 30 points (client-specific productivity metric).
• Floating: Candidate must be able to travel within the assigned coverage area between patient homes.
