Founded by two primary care providers in Scottsdale, AZ, the organization has expanded to multiple locations in Arizona. The care model combines a supportive, team-focused clinical environment with a robust telehealth platform. This approach enables patients and clinicians to stay consistently connected, offering flexibility, continuity of care, and improved access to primary and specialty services. Role Description The Primary Care PA‑NP is a full‑time, on‑site role based in Phoenix, AZ, providing comprehensive primary care to a diverse patient population. Day‑to‑day responsibilities include conducting patient assessments, taking medical histories, performing physical exams, ordering and interpreting diagnostic tests, and developing evidence‑based treatment plans. The PA‑NP will manage chronic and acute conditions, provide preventive care, deliver patient education, and coordinate care with physicians and other members of the clinical team. The role also involves documenting encounters in the electronic health record, supporting quality improvement initiatives, and collaborating with the telehealth team to ensure continuity of care across in‑person and virtual settings. Qualifications Candidates should possess strong Personal Assistance skills, with the ability to support clinical workflows and patient needs efficiently. Candidates should possess Executive Administrative Assistance and Clerical Skills to manage clinical documentation, patient records, and practice operations effectively. Candidates should possess excellent Communication skills and Diary Management capabilities to coordinate appointments, follow‑ups, and care plans with patients and the care team. Current, unrestricted Nurse Practitioner or Physician Assistant license in Arizona; national certification (e.g., AANP, ANCC, NCCPA) preferred. Experience in primary care or family medicine, including management of common chronic and acute conditions in diverse populations. Proficiency with electronic health records and comfort working within integrated on‑site and telehealth care environments. Demonstrated ability to work collaboratively in multidisciplinary teams and deliver patient‑centered, culturally sensitive care.
