Responsibilities Provides a well-focused psychosocial and discharge planning assessment of patient and family to develop a treatment plan and goals, appropriate to the age of the patient. In collaboration with the team, contributes to defining the need for further resources or services. Implements the treatment plan to achieve the treatment goals relative to age specific needs. Completes discharge planning and psychosocial assessment resulting in identification of the patient’s current and, where appropriate, past life crisis and probable discharge plan needs. Completes appropriate intervention plans on the interdisciplinary treatment plan within 72 hours. Coordinates with the treatment team in facilitating an optimal discharge plan for patients, including knowledge of general and age-specific resources including availability, eligibility, cost factors and types of service. Standards and Professional Practice Engages and communicates with patients and families in a direct, empathic manner, adapting approaches to age-specific sensory, cognitive, and social needs. Gathers, assesses, and interprets psychosocial data with awareness of age-specific factors such as signs of abuse, educational needs, and functional deficits. Advocates and communicates the family’s expectations and perspectives and the impact of stress and illness on their functioning. Identifies and contributes knowledge of the impact of ethnic, cultural, socioeconomic, gender, and sexual orientation factors on treatment and well-being. Demonstrates knowledge of different therapy modalities appropriate to the patient’s age and contributes to treatment and discharge planning in written and verbal form. Demonstrates ability to communicate data to the treatment team clearly and concisely; participates in team meetings and huddles as needed. Maintains current knowledge of community resources and provides accurate information to staff, patients, and families (including limitations, costs, and availability). Education and Experience Education - MSW : MSW or master’s degree in counseling. Training and Experience : Training and clinical experience in behavioral health; case management experience. Education - BSW : Bachelor’s degree in social work or other behavioral health specialty. Training and Experience : Training and clinical experience in behavioral health; case management experience. Mental Demands : Able to tolerate the stress of a fast-paced environment and work with multi-disciplinary staff and difficult patient populations.
