New Graduate Nurse Practitioner Fellowship

Oak Street Health

Oak Street Health is seeking New Graduate Nurse Practitioners for our structured Fellowship in primary care for adults on Medicare. Fellows deliver team-based, patient-centered care in underserved communities while receiving intensive mentorship, protected learning time, and precepted clinics. You’ll perform assessments, manage chronic and acute conditions, promote preventive health, and document in our EHR. In our inclusive, collaborative culture, you’ll grow clinically and professionally, help drive innovative care models, and make a meaningful impact on community health in a supportive environment focused on development and equity.
