Description: The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care. Utilizes the nursing process based on theory, research, evidence-based practice, and approved organizational nursing standards. Assesses and evaluates patient care needs while applying critical thinking skills in patient care management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. Prioritizes all aspects of patient care, including teaching, rounding, coaching, and planning after hospital care. Educates patients and caregivers about their plan of care, health promotion, and disease prevention. Communicates effectively and professionally with patients, families, and all members of the interdisciplinary patient care team. Manages assignments within the care delivery model. Role models the tenets of Relationship Centered Care through facilitation of actions such as admission welcome and bedside shift report. Considers patients' values, preferences, and cultural diversity in all aspects of care. Establishes and maintains therapeutic relationships with patients and families. Uses evidence-based practices to enhance understanding of patients' perceptions of care. Initiates service recovery for patients and families with concerns, escalating issues as needed. Demonstrates caring, respect, empathy, and active listening through dialogue and actions. Conducts purposeful rounding on assigned patients. Practices safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement. Demonstrates clinical skills and knowledge crucial to quality and safety in the patient population served. Implements process improvement strategies based on evidence and procedures. Engages in peer and institutional review processes. Collaborates on quality improvement projects, evidence-based practice activities, and nursing research studies. Contributes to quality and safety practices and compliance measures. Implements high reliability principles to ensure a safe environment for patients. Escalates potential safety hazards or gaps from best practice as appropriate. Utilizes information technology to document nursing practice. Performs charge nurse duties as assigned. Coordinates with management to ensure supplies are utilized properly and inventory is sufficient for patient care. Escalates material or staffing resource deficits as necessary. Documents accurately in compliance with hospital requirements and standards of care. Facilitates patient throughput on a shift basis. Contributes to the care environment to support clinical colleagues, patients, and their families. Advances clinical competence in nursing practice. Projects a professional image and communicates with styles that demonstrate respect and compassion. Incorporates ethical principles into decision making for patients and families. Encourages a spirit of scholarship, inquiry, and lifelong learning. Acquires knowledge and skills relative to the role and patient population. Participates in shared decision-making through councils and committees.
