A-Line Staffing Solutions is hiring Case Management Coordinator professionals for a field-based Case Management opportunity in Madison County, Illinois! This is a great opportunity for a case management professional who enjoys coordinating care, supporting members, and making a direct impact on healthcare outcomes.

Pay & Schedule

• $34.13/hour
• Monday–Friday, 8:00 AM–5:00 PM
• Remote/field-based position
• 50–75% travel required to meet with members face-to-face
• Quarterly in-person member visits required
• Reliable transportation required
• Mileage reimbursement available according to company policy

Responsibilities

• Conduct comprehensive evaluations of members’ needs and eligibility
• Identify high-risk factors and service needs that may impact member outcomes
• Develop, coordinate, and monitor member care plans
• Coordinate healthcare, community, and support services
• Manage a full caseload of waiver/LTSS members
• Collaborate with case managers, Medical Directors, supervisors, and other healthcare programs
• Identify and escalate quality-of-care concerns
• Use motivational interviewing and negotiation skills to engage members
• Provide coaching, education, information, and support to members
• Help members actively participate in healthcare decision-making
• Monitor member progress and adjust care plans as appropriate
• Maintain accurate documentation in accordance with company, regulatory, and accreditation requirements
• Conduct required face-to-face member visits

Requirements

• Must reside in Illinois within the Madison County area
• Valid, unrestricted Illinois Driver’s License
• Reliable transportation
• Willing and able to travel up to 75% of the time
• 2+ years of case management experience
• Experience with Microsoft Office
• Experience working with electronic health records
• High School Diploma or GED required
• Bachelor’s degree or Master’s degree in a human-services field such as Sociology, Special Education, or Rehabilitation Counseling
