Eleanor Nursing Care Center , a premier skilled nursing facility in Hyde Park, NY, is seeking a full-time Social Worker to join our dedicated team of healthcare professionals. If you’re passionate about supporting residents and their families while working in a collaborative, compassionate environment, we’d love to hear from you.
Position Overview
The ideal candidate will have experience in a Long-Term Care (LTC) or Sub-Acute setting and a strong commitment to enhancing the social and emotional well-being of our residents.
Key Responsibilities:
- Conduct initial psychosocial assessments upon resident admission
- Coordinate discharge planning for short-term residents, including documentation and maintaining the facility discharge calendar
- Identify residents’ social and emotional needs and develop appropriate care plans
- Complete MDS-related assessments, including social work sections
- Schedule, document, and lead resident care conferences
- Participate in interdisciplinary care plan meetings to ensure comprehensive resident care
Requirements:
- Prior experience in Long-Term Care or Sub-Acute settings required
- Strong organizational, communication, and interpersonal skills
