Branch Coordinator, Home Health

The Medical Records Clerk 2 assembles and maintains patients' health information in medical records and charts. This position performs varied activities and moderately complex administrative, operational, and customer support assignments. It typically works on semi‑routine assignments. The primary function of the Branch Coordinator is to provide clerical support for the branch medical records and billing department, and to manage day‑to‑day telephone and personnel communication systems, agency supplies, mail, and accounts payable. Depending on branch census, additional duties may be assigned. Essential Functions: Prepare patient assessment packets, including admission, recertification, and post‑hospital packets. Process signed and unsigned orders and 485s. Follow up on the Order Tracking Report weekly according to Order procedure, and reprocess unsigned orders at end of episode as tasks appear on the action screen. Date stamp all received documents, except SOC paperwork and paper visit notes, to be scanned into the client chart within 24 hours of stamp date. Fax discharge/transfer summaries, episode detail summaries, and Case Conference/60‑Day summaries to the physician as tasks appear on the action screen. Copy documentation and assist with ADR preparation. Update HCHB and computer programs as appropriate. Complete End of Period claim workflow as part of Billing Specialist responsibilities, confirming all orders are signed and coordinating with the administrative team to ensure billing requirements are satisfied for timely claim release. Adhere to and participate in the Agency’s mandatory HIPAA/Privacy Program and Employee Compliance Program. Read and adhere to all Agency Policies and Procedures and follow Employee Handbook Guidelines. Manage incoming telephone calls professionally, including screening, routing to appropriate agency staff, and maintaining the phone system (add/revise voicemails, maintain accurate employee roster). Process incoming and outgoing mail. Manage and order office supplies, including agency‑specific forms. Manage, order, and distribute medical supplies as needed. Assist BD with EOE billing tasks. Process all accounts payable, routing to appropriate corporate department as needed. Knowledge/Skills/Abilities: Understand issues related to the delivery of home health services and solve problems effectively. Possess knowledge of Medicare guidelines governing home health and hospice agencies. Demonstrate exceptional customer service skills and communicate effectively with referral sources, patients, families, and agency employees. Be organized, detail‑oriented, and possess effective communication skills both orally and in writing. Communicate with a diversity of individuals is required. Education: High school diploma or equivalent, typing and clerical skills, and competence with computers. Experience: Minimum of two years’ experience in the health care industry and one year experience in home health. Desired: Six months of medical records experience in a clinic, hospital, or home health setting (preferred).
