We are seeking a compassionate, motivated Family Nurse Practitioner (FNP) to provide high-quality, patient-centered primary care to individuals and families across the lifespan. This role is ideal for a clinician who is adaptable, collaborative, and committed to delivering culturally sensitive services in a friendly, professional setting. Key Responsibilities Provide comprehensive primary care services across the lifespan, including assessment, diagnosis, treatment, and follow-up care Prescribe medications and manage treatment plans within scope and full prescribing authority Document care accurately and timely in the electronic health record (EHR) Collaborate with physicians, nurses, behavioral health providers, and other clinical team members to support quality patient outcomes Deliver exceptional patient experience by consistently maintaining a friendly, professional, and public-facing presence Support clinic workflows and contribute ideas to improve care delivery, efficiency, and patient satisfaction Maintain compliance with organizational policies and applicable state/federal requirements Required Qualifications Minimum 1 year of experience practicing as a Family Nurse Practitioner (relevant prior medical/clinical experience considered) Completion of an accredited Nurse Practitioner program Current, unrestricted Arizona APRN license as a Certified Nurse Practitioner (CNP) with a Family/Individual Across the Lifespan focus and full prescribing authority Board Certified as a Family Nurse Practitioner (AANP or ANCC) Initiative, creativity, problem-solving ability, adaptability, and flexibility Ability to collaborate with professional colleagues to provide quality care Consistently present a friendly, welcoming, and professional public-facing presence Strong verbal, written, and communication skills Knowledge of Windows environment and/or Electronic Health Records (EHR) systems Ability to provide culturally sensitive services Must have or be able to obtain a valid Fingerprint Clearance Card Must possess reliable transportation, a valid driver’s license, and proof of auto insurance BLS certified or ability to be trained within 5 business days of hire date Experience working in community health, integrated care, or with diverse/underserved populations preferred Pre-Employment / Onboarding Requirements TB screening record and PPD/QuantiFERON/Chest X-ray upon hire (as applicable) Why Join Us Supportive, collaborative clinical team Opportunity to make a meaningful impact in the community 401(k) 401(k) matching Medical Insurance Vision Insurance Paid Life Insurance Paid Disability Insurance Paid time off Employee assistance program Referral program *** NHSC Loan Repayment Program - awards up to $50,000 in exchange for a two-year commitment to provide primary medical, dental, or mental/behavioral health care at approved sites in high-need, underserved areas.
