Job Summary:

We are seeking a dedicated, compassionate, and certified Primary Care Physician Assistant (PA-C) to join our growing healthcare team. In this role, you will provide comprehensive medical care, manage chronic and acute conditions, and emphasize preventative wellness for a diverse patient panel. You will work collaboratively with our supervising physicians and clinical staff to deliver high-quality, patient-centered care. We operate outpatient locations. Our model emphasizes quality metrics , coordinated care, and strong patient continuity.

We are seeking a motivated, patient-centered provider who values autonomy, teamwork, and high-quality medicine.

Position Overview:

• 1 year of experience.
• Preferred 2+ exp. 
• Medical Spanish preferred.
• Family Nurse Practitioner (FNP).
• Certified Nurse practitioner (CNP).
• Physician Assistant (PA-C).

Key Responsibilities:

• Conduct comprehensive physical exams and patient assessments.
• Diagnose and treat acute and chronic illnesses.
• Order, interpret, and manage diagnostic tests.
• Formulate and implement personalized treatment plans.
• We are a value-based care practice focused on preventative medicine, chronic disease management, and long-term patient relationships.
  

Full-time preferred. Part-time considered 3+ days

Responsibilities:

• Provide comprehensive primary care to adult and geriatric patients.
• Manage chronic conditions (HTN, DM, CHF, COPD, etc.).
• Conduct annual wellness visits.
• Meet quality metrics aligned with HEDIS/value-based contracts.
• Collaborate with supervising physician and care team.
• Document efficiently in EMR.
  

Ideal Candidate:

• 2+ years primary care experience.
• Comfortable in value-based/capitated model.
• Strong communication skills.
• Efficient but compassionate.
• Spanish speaking preferred.
