Field-Based Community Health Navigator - NJ (Full-Time)

MedZed seeks a Community Health Navigator (CHN) for the Complete Connect Program in New Jersey. This full-time, field-based role focuses on finding, enrolling, and supporting Medicaid members throughout a six‑month enrollment, with strong emphasis on outreach and follow-up.
