Compassionate Hospice RN Case Manager

Gentiva—SouthernCare is seeking a Hospice RN / RN Case Manager to lead in-home nursing care across patient homes and community settings. You will assess, plan, and deliver skilled nursing care, coordinating with physicians, LPNs, CNAs, social workers, and chaplains to support patients and families through end-of-life care.
