Home Health Aid/Certified Nurse’s Assistant

Objectives The Home Health Aide (HHA) / Certified Nursing Assistant (CNA) is a qualified professional who provides home services which include personal care, homemaking, and companionship that is in compliance with the state Practice Act, any applicable licensure requirement, applicable healthcare standards, state and federal laws and regulations, THE ROADS HOME HEALTH CARE, INC. policies and standards, and philosophy of care. The HHA / CNA is responsible to proactively apply professional knowledge and skills to meet the needs of the patient and facilitate the achievement of individualized, defined and measurable outcomes. OSHA defines this position as Class I exposure to blood. Organization This position reports and is supported by the Director of Nursing, or a Registered Nurse designee. During a facility assignment, the HHA / CNA has an additional responsibility to both facility-designated supervisor and all facility practices, which exceed those of the company. Responsibilities and Duties The HHA / CNA provides home health services interventions in the treatment of the patient illness, rehabilitative needs and preventative care. He/she utilizes a holistic approach in the provision of personal care, homemaking activities, and companionship contained in a written assignment by a licensed professional employee or contractor of Care Tech Home Health Services, LLC. He / She uses a sympathetic attitude towards the sick, is able to read, write and carry out directions, and maturity and ability to deal effectively with the demands of the job. The HHA / CNA is responsible to provide service in accordance with accepted standards of clinical practice, to facilitate problem-solving, and thereby, the achievement of individualized patient goals and outcomes. Personal care activities, which include personal hygiene (bed bath, shower bath, sponge bath), hair (shampoo, brushing, combing), oral hygiene, nail (cleaning and filing and not cutting), skin, shaving, and dressing. Safe transfer techniques and ambulation Other activities as taught by a licensed health professional employee or contractor of this agency for a specific patient may include but not limited to: a. Assisting with devices for aid to daily living such as wheelchair, walker; bedside commode and always maintaining body alignment b. Assisting with prescribed range of motion exercise; c. Assisting with the change of colostomy bag, reinforcement of dressing; d. Assisting with prescribed ice cap or collar; e. Measuring temperature, pulse, and respiration f. Measuring intake and output of fluids g. Doing simple urine test for sugar, acetone, or albumin Maintenance of a clean, safe, and healthy environment, which may include light cleaning and straightening of the bathroom, straightening the sleeping and living areas, washing the patient’s dishes and laundry Preparation of meals based on specific diet requirements by professional personnel, serving, and feeding. Light shopping and errands Documentation The HHA / CNA will prepare and maintain complete documentation as required by the Agency. The HHA / CNA will submit complete documentation in a timely manner as defined by the Agency’s policy. NOTE: Submission of clinical, time visit notes, and any other documentation related to documents is required for satisfactory completion of an assignment as defined by the Agency’s policy. Failure to provide required completed documentation and submitting documentation on a timely basis as defined by the Agency’s policy will result in disciplinary action, and on the case of time documents, the Agency will be unable to generate a paycheck due to the lack of written documentation of time worked. The HHA / CNA Report changes in the patient’s condition and needs immediately to the Registered Nurse. Observed appearance and behavior changes in the patient and reports it to the Registers Nurse immediately. Maintains the highest standards of professional conduct in relation to information that is confidential in nature. Shares information only when the recipient’s right to access is clearly established and the sharing of such information is clearly in the best interest of the patient. Recognizes emergencies and has knowledge of emergency procedures. Contacts the office / DON immediately with any itinerary changes. Can be contacted at all times by beeper / cellular while in the field. Professional Conduct and Development Participates in regular conferences with the Registered Nurse and other care staff in regard to patient care. Attends and participate in continuing education and meetings as required by Agency’s policy and regulations. Participates in periodic review of the Home Health Agency Program, policies, and future considerations. Meets and updates credentials/certificates and documents as per the requirement of the Agency. Maintains a professional appearance as per dress code guidelines. Conducts self according to accepted standards of professional practice in the course of assigned duties are reliable, punctual, and cooperative. Performs other duties as assigned in response to the patient, office, and/or facility needs.
