Ultimate Therapy is seeking a Physical Therapist (PT) to join the rehabilitation team at Autumn Lake at New Britain. This role offers the opportunity to provide individualized care while helping residents improve movement, rebuild strength, and work toward greater functional independence.
What You'll Do

• Complete physical therapy evaluations and develop individualized plans of care
• Provide skilled treatment focused on strength, balance, gait, transfers, endurance, and mobility
• Establish functional goals and monitor resident progress throughout treatment
• Adjust treatment approaches based on clinical findings and resident response
• Educate residents and caregivers on exercises, mobility strategies, and safety
• Maintain accurate and timely clinical documentation
• Collaborate with PTAs, OT, SLP, nursing, and other members of the interdisciplinary team

Qualifications

• Active Connecticut Physical Therapist license required
• Graduate of an accredited Physical Therapy program
• Strong clinical judgment, communication, and teamwork skills
• Skilled nursing or rehabilitation experience preferred, but not required
• New graduates are welcome to apply
