Job Title
Position at Western CT Home Care Inc
Responsibilities
- Performs physical therapy assessment including functional limitations, rooms, capabilities, mobility status, endurance status, ADL status.
- Identifies patient and family needs for other home health services
- Provides services that are ordered by the physician as indicated in the plan of care.
- Provides appropriate treatment.
- Records on a visit basis, all evaluation data, treatments and client's response to therapy, provides client's discharge planning.
- Participates in case management conferences with team members to coordinate physical therapy and total plan of care.
- Provides counseling and education to patients, family and other health care personnel
Required
- Bachelor's degree
- Minimum Experience: One (1) year
- Current Physical Therapist license
