Mental Health Case Manager | Make a Daily Impact

Case ManagerThe H.U.F. Center is seeking a compassionate and reliable Case Manager to support adults with mental health and co-occurring disorders in a psychosocial day support setting. As a Case Manager with Hope, Unity, and Freedom (H.U.F.) Center, you play a vital role in supporting adults with primary mental health diagnoses, co-occurring disorders, and/or substance use challenges. This position focuses on providing psychosocial day support services that promote independence, stability, and overall well-being. This is a hands-on, highly interactive role where you'll work closely with members throughout the day—providing direct support, facilitating interventions, and ensuring all services are clearly and accurately documented.
