Working within the Patient Aligned Care Team (PACT) model, you will evaluate, diagnose, treat, and manage a wide range of acute and chronic medical conditions while promoting preventive healthcare and wellness. This role offers the opportunity to make a lasting impact on the health and well-being of those who have served our country.Schedule: Mon-Fri, 8:00am-4:30pmWhat You'll DoProvide comprehensive outpatient primary care, including preventive, acute, and chronic disease managementPerform patient assessments, medical histories, physical examinations, and develop individualized treatment plansOrder, interpret, and communicate laboratory, imaging, and diagnostic test resultsPrescribe medications and monitor treatment effectiveness in accordance with VA guidelinesCoordinate referrals and collaborate with specialists and multidisciplinary healthcare teamsPromote preventive health through screenings, immunizations, wellness counseling, and patient educationMaintain accurate and timely documentation within the VA electronic health recordParticipate in quality improvement initiatives and maintain compliance with VA policies, patient safety standards, and privacy regulationsFoster effective communication with veterans, their families, and the healthcare team to ensure continuity of careComplete required VA credentialing, privileging, mandatory training, and continuing medical education requirementsRequired QualificationsActive, unrestricted physician licenseBoard certified or board eligible in internal or family medicineCurrent DEA registrationNational Provider Identifier (NPI)Current American Heart Association (AHA) Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certificationsSuccessfully complete the AMI credentialing processIf you're looking for an opportunity to provide exceptional care while honoring those who have served our country, we'd love to hear from you.
