COTA Occupational Therapist Assistant

We seek a highly skilled and experienced Registered Nurse with specialties in Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, Operating Room. The Registered Nurse will be responsible for providing skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. Daily tasks include patient assessments, medication administration, treatment planning, and collaborating with the interdisciplinary team. Strong communication and collaboration with patients, providers, and other team members are critical. At least 2 years of experience and exceptional skill is required. Responsibilities:

• Provide skilled nursing care, including patient assessments and medication administration.
• Develop and implement treatment plans tailored to patient needs.
• Collaborate effectively with interdisciplinary team members to ensure comprehensive patient care.
• Manage case coordination and hospice care for patients with serious illnesses.
• Maintain accurate and timely documentation of patient status and care delivered.
• Communicate clearly with patients, families, and healthcare providers regarding care plans and progress.

• Active Registered Nurse license in the state of practice.
• Bachelor's degree in Nursing or related field.
• Minimum of 2 years experience in hospital, home health, or hospice care.
• Clinical assessment, medication administration, and treatment planning skills.
• Strong interdisciplinary team collaboration skills.
• Excellent communication and interpersonal skills.
• Driver's license, car insurance, and automobile for field work.

• Experience in specialized nursing areas such as ICU, ER, Psychiatric, Labor & Delivery, PACU, or Operating Room.

• Ability to work in fast-paced, high-acuity healthcare environments.
• May require flexibility for on-call duties or variable shifts depending on patient care needs.

• Typically assigned shifts based on facility requirements; may include day, evening, or night shifts.
• Scheduling may be influenced by patient census and care demands.
