Hopestone Hospice RN Case Manager - East Houston Full-Time, Houston, TX, US Join Hopestone Hospice and make a meaningful impact where it matters most—providing compassionate end-of-life care to patients and families in the comfort of their homes. As a Hospice RN Case Manager, you will lead and coordinate individualized patient care plans while delivering expert clinical support, comfort, education, and advocacy. You’ll partner with an interdisciplinary team to ensure every patient receives dignified, high-quality care throughout their hospice journey.
