Nurse Practitioner - Cardiology

Columbia University Irving Medical Center, Department of Medicine, is seeking a full-time Nurse Practitioner (NP) specializing in cardiovascular patient care for a ColumbiaDoctors multi-specialty practice. The NP will practice under the management of the Chief Nurse Practitioner and direction of the CDHV/CDMG Medical Director within the Division of Cardiology. The NP will work collaboratively with physicians, APP peers, registered nurses, other members of the care team, and administrative support staff to ensure exceptional patient care.

Responsibilities

• Perform and document age-appropriate comprehensive medical, surgical, and psychosocial health histories, physical examinations (document pertinent positive and negative findings), and initiate/maintain a problem/medication list.
• Diagnose, treat, and manage chronic and episodic adult patient cardiovascular problems.
• Order and interpret diagnostic tests, prescribe pharmacologic and non-pharmacologic treatments, and provide point of care testing, vaccinations, and medication administrations for cardiovascular patients.
• Perform cardiac testing to include stress testing, device interrogations, bubble study, and echo with contrast studies.
• Manage remote cardiac device clinic and anticoagulation clinic.
• Perform pre-op cardiac clearance evaluations and provide post-op follow-up care.
• Complete appropriate health screenings and health data collection per protocols.
• Coordinate, monitor, and deliver care to patients collaboratively and/or autonomously as per licensure.
• Obtain and review internal and external records/reports.
• Formulate a goal-directed plan that includes appropriate medical and nursing interventions according to established protocols and application of evidence-based care.
• Follow-up/collaborate with the physician/specialist to re-develop the treatment plan as necessary.
• Counsel/educate patients and their caregivers on health maintenance, anticipatory guidance, disease prevention, lifestyle modifications, chronic and acute health conditions, and comprehensive plan of care.
• Perform cardiovascular assessments and complete patient transitions of care and discharge assessments from acute and long-term care facilities.
• Work with clinical team to triage daily patient calls and communicate with referring providers.
• Work with multidisciplinary teams including behavioral health, social work, nutrition, pharmacy, home care, physical rehabilitation, hospice and other specialists as necessary to provide/coordinate care across the continuum.
• Document all patient/caregiver encounters in the outpatient EMR.
• Support clinical team and participate in team meetings and utilize leadership skills to promote established clinical workflows, teamwork, and excellent customer service.
• Assist with clinical staff skills training and in-services and ordering supplies.
• Demonstrate responsibility and accountability for Nurse Practitioner practice within respective scope of care and the delivery of safe, competent patient care.
• Participate in maintaining a safe, private, empathetic, holistic, patient/family-centered environment.
• Respond to medical emergencies.
• Portion of practice may be spent at other ColumbiaDoctors sites.
• Perform other responsibilities as assigned.

Minimum Qualifications

• Requires a Master's degree in Nursing or equivalent in education and experience, plus one to two years of experience
• Current New York State Nurse Practitioner licensure.
• NP board certification.
• Must be eligible for DEA certification.
• Must be eligible for American Heart Association (AHA) ACLS and BLS.
• Familiarity and proficiency with computer-based programs such as electronic medical records systems, MS Word, MS PowerPoint, and MS Excel.

Preferred Qualifications

• At least three years of experience managing adult patients in a cardiovascular setting.
• At least two years of prior clinical experience in a hospital or relevant setting.

Other Requirements

• Participation in Medical Surveillance required:
  • Contact with patients and/or human research subjects
  • Wear N-95 respirator
  • Potential bloodborne pathogen exposure
  • Work with known infectious agents or biological toxins where prophylactic vaccination is available
• Successful completion of applicable compliance and systems training requirements.
