Physical Therapist

Overview Physical Therapist Assistant position with Centralia Assured Home Health, part of LHC Group. The role supports patients in home health settings under the supervision of a Physical Therapist, focusing on treatment, exercise instruction, and patient education to improve well-being. Responsibilities Follow state-specific laws governing home care physical therapy and perform only procedures defined by the supervising PT; communicate recommendations for changes in the plan of care to the supervising Physical Therapist. Instruct and assist patients with active and passive exercises, muscle re-education, gait, functional activities, ADL, transfers, safety, and prosthetic training. Observe, document, and report patient responses to treatment and changes in condition to the supervising PT, nurse supervisor, and/or physician; coordinate care with the healthcare team as appropriate. Educate patients, families, caregivers, and other health team members on therapy areas within the scope of the physical therapy assistant. Required Qualifications Current, licensed, registered, or certified to assist in the practice of physical therapy in the state of Washington. Current driver s license and vehicle insurance, access to a dependable vehicle, or public transportation. Ability to function in various home environments regardless of age, race, creed, color, sex, disability, or financial condition of the client. Preferred Qualifications Ability to perform physical tasks required for patient care, including lifting, transferring, and mobility assistance. Pay and Benefits Pay ranges: Annual Total Cash Target Pay $66,977 - $100,465; Hourly Pay Range $32.20 - $48.30; Per Visit Point Pay Range $38.64 - $57.96. Total compensation may include earnings from per visit point pay and hourly pay based on productivity and performance. Benefits include a comprehensive benefits package, equity stock purchase, and 401k contribution, subject to eligibility. The role may involve two types of compensation depending on the work performed (visit-based or non-visit activities). We comply with applicable minimum wage laws.
