Role Description

This is a full-time, on-site Pharmacist role based in the Cincinnati Metropolitan Area. The Pharmacist will review and verify medication orders for safety, accuracy, and appropriateness, ensuring alignment with clinical guidelines and patient-specific factors. Daily responsibilities include dispensing medications, monitoring for drug interactions, advising patients and clinicians on proper use, dosage, and side effects, and supporting medication therapy management. The role involves collaborating closely with physicians, nurses, and other healthcare professionals to optimize patient outcomes, as well as maintaining accurate documentation and complying with regulatory, safety, and hospital policies. The Pharmacist will also participate in quality improvement initiatives, inventory control, and ongoing professional education to support best practices in patient care.

Qualifications

• Demonstrated clinical pharmacy expertise, including medication order verification, drug interaction screening, and therapeutic drug monitoring.
• Strong patient counseling and communication skills to explain medication use, side effects, and safety considerations in an accessible manner.
• Ability to collaborate effectively with interdisciplinary care teams, including physicians, nurses, and allied health professionals.
• Proficiency with pharmacy information systems, electronic health records, and medication distribution technologies.
• Thorough knowledge of pharmacy laws, regulations, formulary management, and hospital policies related to medication use.
• Doctor of Pharmacy (PharmD) or equivalent degree from an accredited institution.
• Current, active pharmacist license (or eligibility for licensure) in the applicable state.
• Hospital or health-system pharmacy experience preferred; residency training (PGY1/PGY2) is a plus.
• Commitment to evidence-based practice, continuous learning, and participation in quality and safety initiatives.
• Ability to work on-site in a fast-paced clinical environment, with strong attention to detail and sound professional judgment.
