Job Overview

Comfort Creek Nursing & Rehab Center is seeking a dedicated and compassionate Physical Therapist Assistant (PTA) to join our in-house rehabilitation team . We are currently hiring for both Full-Time and Per Diem (PRN) positions. The PTA will work under the direction and supervision of a licensed Physical Therapist to provide high-quality rehabilitation services to residents in our skilled nursing and subacute rehabilitation setting.

This role offers an opportunity to make a meaningful impact by helping residents regain strength, mobility, independence, and quality of life following illness, injury, surgery, or hospitalization.

Essential Duties and Responsibilities

• Administer physical therapy treatments and interventions as outlined in the Physical Therapist's plan of care.
• Assist residents in improving mobility, strength, balance, endurance, and functional abilities.
• Monitor resident response to treatment and communicate progress or concerns to the supervising Physical Therapist.
• Accurately document treatment sessions, patient performance, and progress toward goals in accordance with facility and regulatory requirements.
• Collaborate with interdisciplinary team members including nursing, therapy, social services, and physicians to optimize resident outcomes.
• Educate residents, families, and caregivers regarding therapy programs and safety recommendations.
• Participate in care plan meetings and discharge planning as appropriate.
• Maintain compliance with Medicare, Medicaid, state, federal, and facility regulations.
• Support a positive, patient-centered rehabilitation environment.

Qualifications

Required

• Graduate of an accredited Physical Therapist Assistant program.
• Current Physical Therapist Assistant license in the State of Georgia.
• Current CPR certification or willingness to obtain upon hire.
• Excellent communication, organizational, and interpersonal skills.
• Ability to work effectively as part of a multidisciplinary rehabilitation team.

Preferred

• Experience in skilled nursing facilities, subacute rehabilitation, long-term care, or post-acute care settings.
• Familiarity with Medicare documentation and reimbursement guidelines.
• Experience treating geriatric and medically complex patient populations.
