Role: Licensed Occupational Therapist providing personalized, in-home and senior living care to older adults, using the Geriatric House Calls™ model. Responsibilities Conduct evaluations, develop individualized treatment plans, and deliver evidence-based therapy to support mobility, independence, and wellness. Provide one-on-one therapy sessions in private residences, senior living communities, and virtually when appropriate. Document patient progress accurately within electronic medical records, adhering to regulatory standards. Collaborate with interdisciplinary teams including PTs, SLPs, and fitness specialists. Educate patients, caregivers, and staff to promote safety and aging in place. Incorporate telehealth into treatment schedules for enhanced care delivery and flexibility. Qualifications & Skills Valid Occupational Therapy license or eligibility in the practice state. Degree from an accredited OT program. Proficiency with computers and EMR systems. Current CPR certification. Physical ability to perform patient transfers, repositioning, and extended walking or climbing stairs.
