About the Role:

• Assist residents with daily living activities, such as bathing, dressing, and grooming, in accordance with their individual needs and care plans.
• Provide emotional support and companionship to residents, engaging in conversations and activities that promote mental and emotional well-being.
• Assist with meal preparation, feeding, and feeding assistance as needed.
• Observe and report any changes in resident condition, behavior, or health to licensed nursing staff.
• Support the delivery of person-centered care by actively listening to and responding to residents' needs and concerns.

Responsibilities:

• Maintain accurate and timely documentation of resident care, including daily logs and incident reports.
• Participate in care conferences and multidisciplinary team meetings to discuss resident care and concerns.
• Effectively communicate with residents, families, and other healthcare professionals to ensure seamless care coordination.
• Maintain a safe and clean environment, adhering to infection control and safety protocols.
