Case Manager

CVS Health

CVS Health Corporation seeks a Field Case Manager Analyst to coordinate care and educate members on health options in the West Side of Chicago. You will develop care plans and work with providers to address barriers and improve care quality. The role requires a bachelor’s degree in behavioral health or a related field, 2+ years of case management and behavioral health experience, and willingness to travel up to 75%.
