Physical Therapist – Outpatient | Lincoln City, Oregon | Start 08/24/2026 | 13-week travel assignment | $2,330–$2,389 per week | 36 guaranteed hours per week | Shift: 5x8 Days | Critical Access Hospital outpatient rehab setting

Responsibilities
- Perform initial evaluations and ongoing outpatient physical therapy for patients with mobility limitations and functional decline.
- Develop and implement individualized treatment plans with measurable goals; manage a typical 45-minute treatment session cadence.
- Deliver evidence-based interventions including therapeutic exercise, manual therapy, gait training, balance and vestibular rehab as appropriate, and coordinate with the broader rehabilitation team.
- Educate patients and families on home exercise programs, safety, fall prevention, and equipment use to support continued improvement.
- Document patient progress, update treatment plans, and maintain timely, compliant records in accordance with HIPAA and facility policies.
- Collaborate with care teammates to coordinate care across acute, swing bed, and outpatient services as needed.

Requirements
- Current, unencumbered Oregon Physical Therapy license
- Healthcare Provider BLS (Basic Life Support) certification
- Ability to travel to multiple points of care within the service area
- Willingness to work in an outpatient rehabilitation setting within a rural, critical access hospital
