We are seeking a Part-Time/PRN Speech Therapist (ST) to join our home health team serving Porter County, IN.
One Home Health: OverviewOne Home Health is a full-service, Medicare-certified home health agency licensed in Illinois and Indiana. - Perform in-home speech-language evaluations and skilled treatments
- Assess and treat speech, language, voice, fluency, and swallowing disorders
- Develop and implement individualized plans of care
- Educate patients, families, and caregivers on treatment strategies
- provide care plan guidance to interdisciplinary team members
- Refer to other disciplines as appropriate and document accordingly
- Supervise HHAs when services relate to the speech-language plan of care
- Complete accurate, timely documentation in compliance with agency standards
- Participate in interdisciplinary collaboration as needed
- Licensed Speech Therapist in Indiana
- ASHA CCC-SLP or CFY candidates actively completing supervise experience
- Minimum 1 year of healthcare experience (home health preferred, not required)
- Strong clinical judgement and communication skills
- Ability to work independently in a home-based setting
- Current CPR certification
- Valid drivers license and insured vehicle
