How you’ll contribute A Discharge Coordinator who excels in this role will:Provides aftercare coordination for patients requiring ongoing medical or psychosocial support post-discharge.Creates and maintains updated community resource listings for aftercare services.Actively participates in the treatment planning process by scheduling aftercare appointments identified by the patient’s provider and therapist.Provides patients and families with resource information related to future care needs.Communicates proactively with clients, families, aftercare providers, and referral sources.Drafts comprehensive discharge summaries and contracts for each assigned patient.Collaborates with the interdisciplinary team including physicians, utilization review, and nursing staff.Completes all required hospital and patient documentation thoroughly and in a timely manner.Applies appropriate crisis intervention and de-escalation skills during emergencies.Maintains confidentiality and ensure all HIPAA standards are upheld.Demonstrates working knowledge of clinical regulations including TJC, Medicaid, and state/federal laws.Why join usWe believe that investing in our employees is the first step to providing excellent patient care.
