Physical Therapist ~ PT

Position Overview Stellar Senior Living is seeking a dedicated and compassionate Physical Therapist (PT) to join our Healthcare & Medical Services team, serving residents across our independent living, assisted living, and memory care communities. In this role, you will use your clinical expertise to help seniors maintain and improve their mobility, manage pain, and maximize their independence, directly enhancing quality of life every day. Key Responsibilities Complete thorough evaluations of residents. Develop personalized treatment plans tailored to the unique needs of older adults. Deliver one‑on‑one interventions. Work closely with nursing staff, physicians, and an interdisciplinary care team to coordinate comprehensive care. Contribute to service planning and support safe transitions as residents’ needs change. Emphasize functional outcomes such as safe transfers, balance and gait training, fall prevention, strength and endurance conditioning. Regularly reassess progress, update care plans, and maintain detailed, accurate documentation that meets clinical, regulatory, and organizational standards. Coach residents and their families on home exercise programs, safe mobility strategies, and proper use of assistive devices. Maintain a safe and organized therapy environment, adhering to infection‑control, equipment safety, and risk‑management protocols.
