Key Responsibilities

• Conduct comprehensive patient evaluations and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
• Perform 60-minute initial evaluations and 30-minute follow-up sessions in an outpatient setting
• Complete accurate and timely medical record documentation including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
• Design functional treatment programs focused on progressing patients toward discharge and effective return to community activities
• Delegate and direct discipline-specific therapy services for assigned patients
• Serve as an educator for patients, families, students, and coworkers while actively pursuing personal professional development

Required Qualifications

• Active and unrestricted Physical Therapist license in the state of Colorado
• Adherence to state-specific Physical Therapy Practice Act and all applicable regulatory requirements
• Ability to complete thorough clinical documentation in compliance with state, federal, and intermediary guidelines
• Demonstrated competency in clinical reasoning and development of patient-centered treatment plans and goals
• Commitment to patient and employee safety standards and protocols

Preferred Experience / Skills

• Prior experience in an outpatient physical therapy setting
• Experience supervising or mentoring physical therapy students
• Familiarity with performance improvement methodologies and quality initiatives
