Physician Leader, Primary Care

HarmonyCares seeks a Physician Leader, Primary Care to provide home-based primary and palliative care for medically complex, home-limited adults. You will lead an interdisciplinary team delivering in-home visits, telehealth, and coordinated care management to improve outcomes and reduce avoidable hospitalizations. Responsibilities include panel oversight, clinical decision-making, quality and safety initiatives, and mentoring providers. You will collaborate closely with nursing, social work, and care coordinators, champion a patient-first culture, and help shape best practices in a rapidly evolving, mission-driven organization.
