In-Home Health - Nurse Practitioner or Physician Assistant (Full Time) - Yakima, WA

Overview Signify Health, part of CVS Health®, is seeking a Full‑time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. These evaluations last about 45 minutes and include a medication and medical history review, a physical evaluation, and selective diagnostic tests (e.g., diabetic eye exams, spirometry). The clinician uses an iPad and an intuitive clinical workflow and is not required to prescribe medication, order lab tests, or alter current treatment regimens. In this role, you will Build trusted relationships to help patients achieve better health outcomes. Participate in a national network of clinicians focused on connecting patients to appropriate care. Travel locally and within licensed states as needed and conduct virtual visits when required. Provide all Signify Health services, including Diagnostic & Preventive Services such as spirometry and diabetic eye exams. Work Environment The primary workplace is the private residence of health‑plan members. Clinicians must be able to safely work in homes that may contain pets, secondhand smoke, and other airborne allergens or irritants. Position Details Anticipated weekly hours: 40 Time type: Full time Pay range: $95,738.00 – $206,206.00 (base hourly or annual salary, dependent on experience, education, geography, and other factors).
